Fall Physicals 08/09
High School and Middle School Students

**There will be no make-ups for fall physicals.
**Physicals are provided free of charge to Laurel Highlands Student/athletes.
**All forms must be filled out in their entirety and returned to the nurse's office the day of physicals.
**Forms can also be picked up at the High School Office beginning Monday June 30, 2008.

Please Adhere to the following schedule:
Senior High Students:
- 8:00A.M. to 9:45A.M.---Football and Cross Country Grades 9-12
- 9:45A.M. to 10:30A.M.---Boys and Girls Soccer Grades 9-12
- 10:30A.M. to 11:15A.M.---Volleyball, Golf and Cheerleaders Grades 9-12
Middle School Students:
- 11:15A.M. to 12:30P.M.---Football Grades 7 and 8
- 12:30P.M. to 1:15P.M.---Boys Soccer, Girls Soccer, and Girls Basketball Grades 7 and 8
- 1:15P.M. to 1:45P.M.---Middle School Cheerleaders, Swimming and Cross Country Grades 7 and 8
