**Date & Location:**
Mark your calendars for Monday, August 14th, as our dedicated school nurse will be conducting fall physicals at the middle school. You can find us in the nurse's office, and we'll be available from 9:00 a.m. to 10:30 a.m.
**Cost & Payment:**
The cost for the fall physical is $10.00, and for your convenience, we're accepting checks or money orders. Please make these payable to "Centerville Clinic." We want to emphasize that we won't be able to accept cash, so let's stick to checks or money orders to streamline the process.
**Paperwork Requirements:**
To make sure we can provide the best care for your child, it's essential that all paperwork is filled out in its entirety. This ensures that the physical examination goes smoothly, and we have the most accurate and up-to-date information.
