LHMS Fall Sports Physical Info

Middle School Fall Sports Physicals Scheduled for June 2

Laurel Highlands Middle School will offer sports physicals for fall athletes on Monday, June 2, 2025, in the Middle School Nurse’s Office. This opportunity is for students entering 6th, 7th, and 8th grades who plan to participate in fall sports.

Fall sports at the middle school level include:

Co-ed Cross Country, Football, Co-ed Swimming, Boys Soccer, Girls Soccer, and Softball.

Important Information:

• Date: Monday, June 2, 2025

• Location: LH Middle School Nurse’s Office

• Cost: $10.00 per student (Checks or Money Orders made out to LH Athletics)

• Forms: Physical forms can be picked up at the middle school security desk

• Requirements: Forms must be completely filled out and signed in all designated areas before the day of the physical.

• Insurance: All students must have medical insurance to receive a physical.

  No insurance – no physical.

Physicals Schedule – Monday, June 2

Students will be called down throughout the school day as follows:

• 8:00 AM – 9:15 AM: Football

• 9:15 AM – 10:15 AM: Boys Soccer

• 10:15 AM – 11:15 AM: Girls Soccer

• 11:15 AM – 12:15 PM: Softball, Cross Country, Co-ed Swimming

• 12:15 PM – 12:40 PM: Late Arrivals

There will be no make-up physicals, so students must be prepared on the scheduled date. Remember, no insurance means no physical, and without a physical, students will not be eligible to participate in practice or games.
