UPMC/LHSD Pfizer COVID-19 Vaccination Opportunity
June 16, 2021
In an effort to establish and maintain the health, safety, and welfare of students, staff, and our community the Laurel Highlands School District in partnership with UPMC will be offering a FREE Pfizer COVID-19 Vaccination Clinic for students and others age 12 or older.
DOSE #2 - JUN 16, 2021 12:00PM - 2:00PM @ LH HIGH SCHOOL GYMNASIUM
Any minor wishing to receive the FIRST DOSE MAY DO SO ON JUNE 16th. To receive the vaccination you must complete the following:
#1 Register for the vaccine by clicking this link:
#2 Print the attached COVID-19 Vaccine Form and bring it with you on
June 16th, 2021.
#3 Have a Legal Parent or Guardian be present on the day you receive the vaccine to sign the printed registration form you will bring with you.
Parking will be available in the student parking lot in front of the swimming pool/gym facility, entrance to the gymnasium will be from that location, masking is mandatory.
