Times:
Football - 8:00 a.m. to 9:00 a.m.
Boys and Girls Soccer - 9:00 a.m. to 10:00 a.m.
Softball/Co-ed Swimming -10:00 a.m. to 10:30 a.m.
Co-ed Cross Country/Co-ed Swimming - 10:30 a.m.
Cheerleading -10:30 a.m. to 11:00 a.m.
The cost of a middle school physical is $9.00:
1. Checks or money orders made out to Centerville Clinic
2. NO CASH PLEASE
3. Mask must be worn per DOH guidelines
Please note physical forms can be picked up at the middle school security office from 9:00 a.m. to 1:00 p.m or you can download a copy that is attached.
